North Shore Community Health is a mid-sized but growing community health center, with three locations, Salem, MA, Gloucester and Peabody. We are looking for a mission driven individual to join our team as a Registered Nurse to join our Gloucester office. Under the general direction of the Director of Nursing (DON) and the direct supervision of the (Associate) Medical Director, the R. N., in collaboration with the physician(s) on her team, will works as an integral part of the Primary Care Team. The RN participates in direct and indirect nursing care, population management, provides health maintenance, and teaching and counseling within the scope of practice for an RN. The RN provides supervision for all the medical assistants at the site. The RN is also expected to provide leadership on the floor team, as well as mentorship and guidance. The RN is expected to have particularly close professional ties with the Site Manager (SM) and the AMD. There is a dotted line from the RN to the SM. Although not the designated supervisor, the SM, on a day to day basis, can direct the nurse where the greatest need of the site and the health center lie, especially in regards to logistics.


  • Provides direct supervision to Medical Assistants; managing performance, providing coaching and ongoing support through goal setting, consistent feedback and performance evaluation.
  • Leads the day to day operations of the clinical team, assigning MAs, room distribution, etc .
  • Provides daily floor nursing care including immunization, wound dressing changes, TST application and education, chronic disease (dm, htn, etc...) follow-up appointments, fielding clinical inquiries by patients and occasionally triaging patients.
  • The current Gloucester schedule requires one evening a week, included in the nurses' hours.
  • The position requires one Saturday morning at the Salem walk-in clinic every other month, with the options of working more Saturday mornings. It will occasionally entail covering other sites, as determined by the needs of NSCH as a whole.
  • Participates in analyzing and interpreting data and making informed judgments related to patient care.
  • Participates with nurses and AMD in planning and implementing nursing interventions.
  • Assesses an individual's basic health status and related patient care needs.
  • Medication Refill Review- reviews Refill requests from pharmacy and patients based on medication refill protocol; performs medication reconciliation and attaches pertinent information necessary for clinician to refill if outside the protocol. Fills out pre-authorization forms as necessary.
  • When appropriate evaluates outcomes of basic nursing interventions and initiates changes in care plans.
  • Participates in the health education required by the individual and family, especially focusing on furthering self-management and self-empowerment to maintain an optimal level of health. Optimally, the nurse would develop a group class around either a chronic disease or lifestyle change.
  • Collaborates and communicates with other health care providers to ensure quality and continuity of care. Provide safe, comfortable, therapeutic environment for patients and families.


  • Graduation from an accredited school of practical nursing, current license to practice as a RN in the Commonwealth of Massachusetts.
  • 1-3 years' experience in a community health or ambulatory setting
  • 1-3 years' supervisory experience helpful.
  • Ability to function effectively both as a leader and a member of a team as well a proven ability of working independently.
  • Ability to work in a culturally diverse environment.
  • Excellent communication and interpersonal skills.
  • Current CPR certification. The position normally requires walking, standing, sitting, lifting, reaching, bending, pushing, and pulling. Must be able to lift and support the weight of 50 pounds in handling patients, medical equipment, and supplies. Critical thinking and ability to concentrate. Must be able to recognize and respond quickly to changes in patient and/or unit conditions
  • Preference will be given to bilingual/bicultural individuals who can speak Spanish and/or Portuguese.
  • Experience with electronic health record preferred. Strong computer skills a clear asset.
  • Sensitivity to low income, ethnic minority community.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
